1. $3 - $40/month - Hosting with web design tools that allow you to modify templates and create the web site yourself. These design tools may have limitations (no ecommerce, no meta tags, page names can't be changed) and most likely won't guide you to create an effective site. Normally discount if paid yearly.
2. $45 - $80/month - "Custom" design programs that roll hosting and design services into one monthly or yearly fee. Normally the number of pages is limited (5, 7, 10, 20) and so are what is able to be designed. Most of the time the web designers are customing templates instead of you doing it like in #1 above. Normally discount if paid yearly.
1. Advantages: Web design can be ongoing with changes made as often as you need.
2. Disadvantages: Most of the time you do not own the domain name or the web site. If you stop paying you loose the site and the domain name use
3. Custom Design is where the web designers/developers use templates to start off the site and then customize it for what is needed. 
4. Original Design is where you hire a web designer to create for you a site from the ground up. The design normally includes Search Engine Optimization, functionality analysis and normally produces the most effective web sites.
